Yes. I've worked in advertising for more than 15 years and can assure you that all photos are touched up to remove natural blemishes and beauty marks as well as accentuate various features…especially abs.
As for altered, that doesn't happen every time. But sometimes…mainly to make the model appear taller or thinner. I don't think this guy's package has been altered. He just looks very well fluffed.
